1. Grant of Rights

I, [submitter of form below] hereby grant and authorize Shelly Haines, and its assigns, licensees, and, successors, the irrevocable right to take, edit, alter, copy, exhibit, publish, distribute, and make any lawful use of any and all photos taken or submitted of my pet, use their name, and me (if applicable). This authorization is for any purpose, including, but not limited to, illustration, advertising, and commercial purposes, in any media now or hereafter known, throughout the world, in perpetuity.

2. Relinquishment of Claims and Compensation

I understand and agree that these materials will become the property of Shelly Haines Lemons to Mint LLC/Fetch Worthy and will not be returned. I waive any right to inspect or approve the finished product or the written copy that may be used in connection with the images. I agree to this release without receiving any compensation, royalties, or payment of any kind. I hereby release Shelly Haines Lemons to Mint LLC/Fetch Worthy, its affiliates, from all claims, demands, and causes of action which I, my heirs, representatives, or any other persons acting on my behalf may make concerning the use of these photos.

3. Representations

I represent that I am the sole owner of the pet and have the full authority to enter into this agreement.

4. Use of original image

The owner may reuse the original image.

5. Signature

I have read and understood this agreement and sign it voluntarily of my own free will.
